Diagnostic Approach
When Fantastic Voyage hit theaters in 1966, this science fiction film didn’t set its sights on distant planets, but rather inward: into the human body. Directed by Richard Fleischer, the film was an adventure that combined elements of the Cold War, medical fantasy, and spectacular special effects. The basic premise is as simple as it is effective: a team of scientists and specialists, along with a submarine, is shrunk to microscopic size and injected into the bloodstream of a seriously injured man. There, they are tasked with removing a blood clot in his brain.
With this unusual premise, Fantastic Voyage transformed organs, blood vessels, and cells into an alien landscape. The human body is depicted almost like an unknown planet. The approximately 100-minute film was produced by 20th Century Fox; Richard Fleischer directed, and Harry Kleiner wrote the screenplay based on a story by Otto Klement and Jerome Bixby.
Intravascular Mission
At the center of the plot is scientist Jan Benes, who defected from the Eastern Bloc to the West and possesses crucial knowledge about a new miniaturization technique. After an assassination attempt, he suffers a severe brain injury. A blood clot is located in a spot that cannot be reached by normal surgery.
Consequently, the secret organization CMDF decides to employ a radical procedure. The submarine Proteus and its crew are shrunk to microscopic size and then injected into Benes’ bloodstream. On board are intelligence agent Grant, surgeon Dr. Duval, his assistant Cora Peterson, medical director Dr. Michaels, and pilot Bill Owens.
The biggest problem is time. The miniaturization lasts only about an hour. After that, the crew and the submarine begin to grow again—inside a human body, with deadly consequences. At the same time, the Proteus veers off its intended course. Among other things, the travelers must pass through the heart, lungs, lymphatic system, and inner ear. Oxygen deprivation, strong currents, and the body’s immune response increasingly turn the operation into a struggle for survival.
Added to this is the suspicion that there is a saboteur on board. Eventually, the team reaches the brain and attempts to destroy the clot with a laser. In this way, the film combines medical adventure with a spy thriller: not only does the body itself become the adversary, but so does a human enemy within the group.
Cast Profiles
Stephen Boyd plays Grant, the security agent who serves as the audience’s point of identification. Boyd was already well-known from films such as Ben-Hur and provides a comparatively down-to-earth focal point for this fantastical story. Arthur Kennedy takes on the role of the brilliant surgeon Dr. Duval, while Donald Pleasence, as the nervous Dr. Michaels, creates an atmosphere of mistrust early on.
Raquel Welch received special attention for her role as Cora Peterson. Fantastic Voyage was one of her first major film roles and played a significant part in introducing her to an international audience. According to the American Film Institute, Welch was recommended by the wife of producer Saul David and subsequently signed to 20th Century Fox. For the role, she had to demonstrate, among other things, that she could run down a long flight of stairs quickly and safely—a surprisingly practical requirement for a science fiction film.
Edmond O’Brien plays General Carter, Arthur O’Connell plays Colonel Reid, and William Redfield plays the pilot Bill Owens. The characters are less psychologically developed than in a character-driven drama. Rather, they fulfill clear functions within the mission: surgeon, technician, pilot, security officer, and medical specialist. As a result, the crew comes across almost like a medical response team.
Optical Histology
The real main attraction of Fantastic Voyage is its visual effects. Long before computer-generated imagery, blood vessels, organs, and cells had to be depicted using sets, models, liquids, lighting, matte shots, and optical multiple exposures.
A large version of the Proteus was built for the production. Richard Fleischer described the main model as about 41.5 feet long—more than twelve meters. Several smaller models were also created. For scenes in which the characters “swim” through the body, the actors were suspended by wires above the sets. The work was by no means pleasant: Stephen Boyd is said to have suffered severe bruising; Raquel Welch was at times nearly knocked unconscious by her harness.
The fake blood also required a great deal of ingenuity. To depict the flow of blood, the production team used, among other things, pressure nozzles and mixtures of oil, petroleum jelly, and dyes. White blood cells, in turn, were simulated using specially developed viscous materials. Medical illustrations from UCLA served as a guide for the designers, although the result was deliberately made to be more fantastical than strictly realistic.
The effort paid off. At the 1967 Academy Awards, Fantastic Voyage received five nominations and won two awards: for special effects and for art direction and set decoration in color.
Clinical Side Notes
An interesting detail in film history concerns Isaac Asimov. It is often assumed that Fantastic Voyage is the film adaptation of an Asimov novel. In fact, it was the other way around: Asimov wrote a novelization based on the screenplay. The book was published—and had already been reviewed—before the film premiered in August 1966. This later led to the impression that the novel had come first.
The scale of the production was also remarkable for a science fiction film of the time. The budget was approximately $6.5 million, and the sets and special effects alone are said to have cost more than $3 million. Post-production work on the effects took months.
Contemporary reviews were overwhelmingly positive. The industry magazine Variety particularly praised the lavish production and described the film as an entertaining journey into “inner space”—that is, the interior of the human body. The influential critic Bosley Crowther of the New York Times also reacted enthusiastically, calling the film one of the most colorful and imaginative science fiction productions since Destination Moon.
However, the enthusiasm was not entirely unanimous. The Harvard Lampoon ranked the film among the ten worst productions of the year. Overall, though, the positive response prevailed, and audiences also received the film well.
Cultural Metastases
The most lasting impact of Fantastic Voyage likely lies in its basic concept. The film turned the journey through the human body into an instantly recognizable science-fiction motif. The body was now no longer merely the subject of medical examination, but a world that, in principle, could be explored like a foreign continent.
As early as 1968, an animated television series titled Fantastic Voyage was produced. Later, the motif was repeatedly taken up and frequently parodied in films, television series, comics, and animated productions. A particularly clear example is Joe Dante’s 1987 science-fiction comedy Innerspace, in which a miniaturized pilot is accidentally injected into another person’s body. The film is explicitly inspired by Fantastic Voyage.
From a scientific and technical perspective as well, the concept retains a special appeal to this day. Miniaturized robots, targeted drug delivery, and medical nanotechnology are reminiscent—at least conceptually—of Fleischer’s 1966 fantasy. Of course, the actual process works completely differently, and humans or submarines cannot be shrunk to the size of a cell. Nevertheless, the film has created a powerful image that encourages us to think of medicine not only from the outside but also as a journey into the body.
A scientific study on the depiction of journeys through the body accordingly identifies Fantastic Voyage as a significant template for an entire tradition of this motif in later popular culture.
Final Assessment
Fantastic Voyage is not a scientifically realistic vision of the future. Its miniaturization technology clearly belongs in the realm of fantasy, and anatomy is often subordinated to the demands of suspense and spectacular imagery. Yet this is precisely where the film’s strength lies. Richard Fleischer and his team transformed the human body into a vast science-fiction landscape and combined medical knowledge with adventure, espionage, and visual imagination.
From today’s perspective, some effects are clearly recognizable as models, sets, and optical tricks. Yet this hardly diminishes their significance. On the contrary: these handcrafted solutions demonstrate the ingenuity with which filmmakers created fantastical worlds before the digital era. The two Oscars for Art Direction and Special Effects underscore the impression this work made in 1966.
Sixty years later, Fantastic Voyage is therefore more than just a technical curiosity. The film is one of those science fiction works whose central idea has long since become part of pop culture. It turns a medical emergency into an expedition into the unknown—and proves that a fantastic journey doesn’t necessarily have to take you millions of kilometers into space. Sometimes, a journey through a single artery is enough.
